  • Checklist for Adult Sponsor / Safety Assessment Form (1).-- This form is required for ALL projects and must be completed prior to experimentation.
  • Research Plan (1A) -- This completed form is required for ALL projects. Type or print all information requested. Answer all questions and complete Research Plan Attachment ( see page 31 of MSEF Booklet)
  • Research Plan Attachment -- REQUIRED for ALL Projects. A completed research plan must accompany Research Plan Form (1A). Additional pages may be attached.
  • Research Plan (1A) - TEAM -- This completed form is required for ALL Projects. Type or print all information requested. Answer all questions and complete Research Plan Attachment ( see page 31 of MSEF Booklet)
  • Approval Form (1B) -- This completed form is required for ALL Projects
  • Registered Research Institutional/Industrial Setting Form (1C) (24 kb)--This form must be completed by the scientist supervising the student research conducted in a registered research institution (e.g., university lab, medical center, NIH, SSTP, etc.) or industrial setting. 
  • Qualified Scientist Form (2) (65 kb) --Required for research involving animals, controlled substances and pathogens; may be required for rDNA, tissues, and humans. Must be signed prior to the start of student experimentation. 
  • Designated Supervisor Form (3) (13 kb) --Required if the Qualified Scientist is unable to supervise the experiment OR for projects using hazardous materials or devices. 
  • Human Subjects Form (4A) (21 kb) --IRB Form for approval before experimentation. Required for all research involving humans. 
  • Informed Consent Form*(4B) (21 kb) --Required for all subjects under 18 years of age (see exceptions); required for all projects involving risk; and recommended for all projects involving human subjects. *Use a separate form for each test subject. 
  • Nonhuman Vertebrate Animal Form (5) (22 kb) --Required for all research involving nonhuman vertebrate animals. (SRC approval required before experimentation). 
Human and Animal Tissue Form (6) (20 kb) --Required for all projects using viable fresh tissue, organs, human or animal parts, including blood, blood products, teeth, primary cell cultures, and body fluids (Plant tissue is excluded.) (SRC approval required before experimentation.)
