MSEF2011 Brochure and Application Forms

MSEF2011 Brochure.  The brochure contains detailed instructions, timeline and application forms.

Required Forms: MSEF registration form and fee, abstract and Forms 1,1A + Research Plan, 1B are REQUIRED for all projects by entry deadline.

• Checklist for Adult Sponsor / Safety Assessment Form (1).-- This form is required for ALL projects and must be completed prior to experimentation.
• Student Checklist/Research Plan (1A) -- This completed form is required for ALL projects. Type or print all information requested. Answer all questions and complete a Research Plan.
• Research Plan Attachment -- REQUIRED for ALL Projects. A completed research plan must accompany Research Plan Form (1A). Additional pages may be attached.
• Approval Form (1B) -- This completed form is required for ALL Projects

• Registered Research Institutional/Industrial Setting Form (1C) (67 kb)--This form must be completed by the scientist supervising the student research conducted in a registered research institution (e.g., university lab, medical center, NIH, SSTP, etc.) or industrial setting. 
• Qualified Scientist Form (2) (75 kb) --Required for research involving animals, controlled substances and pathogens; may be required for DNA, tissues, and humans. Must be signed prior to the start of student experimentation. 
• Designated Supervisor Form (3) (46 kb) --Required if the Qualified Scientist is unable to supervise the experiment OR for projects using hazardous materials or devices. 
• Human Subjects and Informed Consent Form (4) (87 kb) --Required for all research involving humans, including use of surveys or questionnaires.  IRB approval required before experiementation.
• Nonhuman Vertebrate Animal Form (5) (70 kb) --Required for all research involving nonhuman vertebrate animals. (SRC approval required before experimentation). 
• Potentially Hazardous Biological Agents Form 6A  Risk Assessment Form (63kb) – SRC/IACUC/IBC approval required before experimentation begins.

• Continuation Projects Form 7 (55kb)
