Registration Form for MSEF2009

There are 2 formats available for the registration form.  The first is the recommended file to use, it is a Microsoft Word based form with text and form checkboxes to complete.  It has been tested with various versions of Microsoft Word but if you have trouble using it, a plain word document has been created and is available for those that the form does not work for.  Forms are due February 2, 2009.
